After you have completed the form and signed it, please make sure to press the Submit button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

Online Payments
We impose a surcharge of 3.00% when paying with a credit card, which is not greater than our cost of acceptance. The adjustment will appear on your receipt. We do not surcharge debit cards. Any purchases made with a debit card or cash will not include a surcharge. If a cardholder’s zip code matches a restricted territory (CT, MA and PR), the surcharge will not be applied.
